Do Not Assume Internal Medical Billing is Your Best Option

The choice to outsource your medical billing can yield substantially better results than the choice to keep billing in-house. Why? Because a properly structured outsourced billing relationship insures that the medical billing company succeeds by making you succeed.

In most situations (and make sure this is the case for your medical billing service) the billing company only gets paid when the practice gets paid while a practice's in-house staff gets paid irrespective of results. There is also an incentive for the billing company to perform better - the better it performs the more it collects for the practice and in turn the more it earns (and vice versa). This is not true for in-house staff whose wages are typically fixed irrespective of performance and quality of work.

This issue, however, is often not fully understood or appreciated by many providers. These providers frequently say: "the staff works directly for me in my office-- they are more loyal and will do a better job and I can see what they are doing". Experience has shown, however, that this is often not true.

I recently spoke with a partner at a busy cardiology practice. While one of the billers was out sick, some paperwork was required and the supervisor went looking for it. When the supervisor opened the missing biller's desk, a stack of unfiled, old claims was discovered. It turned out about $40,000 of them were past timely filling deadlines. They were lost. I repeat-the practice lost $40,000! When the biller returned from her leave, she was "sternly" reprimanded. Let me say it one more time-she was reprimanded. Not fired, but reprimanded. Either way, the practice lost $40,000 in just this one instance alone.

When I asked the doctor why a more severe action was not taken, he explained to me that "we already have staffing problems and did not want to alienate the billing staff any further." The guilty biller was apparently moved to the front-desk role and is now responsible for gathering demographic information and money.

There should be safety nets in place to catch $40,000 in missing claims. So how could this have gone unnoticed until a desk excavation? The office did not track and reconcile charges, payments or write-offs. The doctors had been told that the practice's system could not report at this level. The system, however, indeed had the capability to do this, but the billing staff did not know how to properly use it. Without the the fully aligned incentives of a medical billing company, the investment is often not made to full utilize the capabilities of a practice's medical billing system. $40,000 in missing charges is likely only the tip of the ice berg for this medical practice.

If you select the correct billing company you can avoid nightmare situations like this. Here are some of the key elements you should seek when looking for a medical billing company:

- A fully integrated tracking system (charges by locations/provider and payments by source - lock box, office, PO Box) should be in place and you should have full visibility into the system at all times.

- Your medical billing company should reimburse your practice for what you would have been paid by the payers based on your allowable for any claims that go past timely filing for reasons within the medical billing company's control. What this means is that you never suffer financially if the billing company drops the ball. Try to have your billers reimburse you if they drop the ball.

- The practice should always (24 hours a day, 7 days a week) have access to the medical billing companies system. This allows the practice to see at any time exactly what is happening with their account.

Physicians are working harder for less as costs rise and reimbursements fall. This is exacerbated by selecting a medical billing approach that does not have the proper alignment of incentives to prevent disasters (such as $40,000 in unbilled charges) from occurring.

No - your staff will not work harder for you just because you employ them; and No - the biller who lost you $40,000 will not do any better job collecting money and gathering information from patients. You will probably need to "sternly" reprimand them again.

A high performing medical billing company with complete transparency and full alignment of incentives is the surest path to medical billing excellence and strong financial performance for your practice.

And they place a value on the links that point to your site, referring to the value as a vote. That's how they determine the content of your site. The link words are how they rate your importance and relevance. Links from some sites are rated higher than others. For instance, .gov or .edu are considered trusted sites and are more valuable, and thus will get bigger votes. PageRank sites will also get bigger votes. Basically, the stronger the site, the stronger the vote will be for an equal link from that site.

Most in the SEO industry will agree that .edu links have a higher value with the search engines, but there are always a few who disagree. In fact, it is just not true that .edu links are rated the same as other links by the search engines. By failing to stay present we actually hurt our businesses and by extension our businesses' future growth and expansion. You will be better able to fight back against the recession by staying present and giving your full attention to the "now."

Do One Thing At A Time Really Well

You can only focus on one task at a time. If you are giving your attention to many tasks at once you are spreading yourself too thin and producing inferior results. If you disagree with this statement let's take a quick example to help explain this idea. Have you ever been on the phone with someone who is watching t.v., paying attention to their children or is otherwise working on something? You can tell right? Even if they deny it, you know that they are not giving you their full attention because they respond slower to your questions or answer you with "uh-huh" one too many times. Multi-tasking in the end makes projects take longer to complete and potentially makes your results worse than they would be if you were giving the project your full attention.

Master The Details

Your brain has to pause and switch between neurons to move back and forth between the different projects or tasks that you're working on. If you have enough distractions this process could take you several minutes to complete. This accounts for the sensation of losing your train of thought when speaking or an inability to focus on a task after coming back from a distraction. Start out each day by making a list of the five most critical things you need to accomplish and then start working your list. If you have meetings or other interruptions, come back to your list each time you have quiet time to work. If quiet time is a problem then set up some boundaries with your employees or co-workers. Are you someone that your boss knows will get the job done. Can they rely on you? Do you put their mind at ease? Let's also take this one step further. Can your co-workers count on you? You want to make sure that the people you work with regardless of whether they are in an authority position, know that they can go to you for projects because you will do the job well and you will get it done on time. Missing deadlines affects more than just your project. Down the line that missed deadline or flawed project could cost your company a client or a sale. While the project might not seem important to you at the time. It might have grave consequences for others.

Are you doing your absolute best at work? Can you step it up a notch? People rarely work at full speed all the time. Today the average person in the UK is largely unaware of the importance of plumbing technology, normally taking for granted a high pressure shower and a modern flushed toilet. It is rare that someone in the modern world takes the time to learn about the way plumbing technology has advanced or the history of plumbing in general. This history is interesting and offers some surprising outcomes.

It was during the Greek and Roman empires that plumbing technology first started to evolve. Basic plumbing systems were invented and installed during these empires' rule to make it easier to carry water to and from the public bathing houses that were so popular. It was this necessity that lead to the invention of the aqueduct during the Roman Empire. The aqueduct was the primary mode of plumbing and water distribution from the Roman Empire until the 1800s.

During ancient times, aqueducts were mostly built out of stone or clay while the pipes leading to and from them were fashioned from lead. Modern plumbing, on the other hand, uses vastly different materials. Today, plastic, steel, brass and copper are widely used for the construction of plumbing systems and pipes. Lead is no longer used because it is highly toxic.

The bath houses that the Romans enjoyed are considered the predecessors of plumbing as it currently exists. Originally, public bathing only occurred while the sun was up because the bath water was only replaced once each day. Remember, it was not until long after the Roman Empire fell that bacterium was discovered and the western world learned how diseases were spread with the obvious implications on bathing and personal hygiene. In Roman times, one water change each day was all they thought they needed.

The modern toilet, another staple of modern plumbing systems in the western world, is also derived from ancient times. The western toilet is largely based upon the toilets used in Mohenjo-Darco as long ago as 2800BC. These toilets were made from a wooden seat that was placed on top of a brick pile. In 2800BC only the richest citizens were allowed to use these toilets. It took as long as the middle of the 1800s for the sit down toilet that was so celebrated in ancient Rome to be adopted by western societies.

Once the western world had adopted the sit down toilets and aqueduct structures of the Roman Empire, the plumbing technology surrounding them expanded very quickly. Within one hundred years, plumbing technology and toilets have gone from the aqueducts of the Roman Empire to the modern efficiencies that most citizens in the United Kingdom take for granted these days.

In today's plumbing systems most pipes are kept under the ground and cesspools and open sewage drains have almost been completely eliminated due to environmental and operational considerations. As the western world continues to advance it is highly likely that plumbing and toilet systems will become cleaner and more efficient as well, with further introduction of green technology and improvement in treatment of sewage and contaminated water. - 18098

When I take a close look, I see 2 types of individuals in this industry. You have the ones that execute on a daily basis, and the ones that don't bother. The ones that will be most successful in networking, building their home business, are the ones that execute.

Friends of mine, and many others I'm associated with, noticed that over the years there is one trait that has allowed me ongoing success. I'm not any more brilliant than the next guy, or have had a ton of luck with opportunity, and certainly not the case of having a lot of money to get going with.

They all say it comes down to one thing. "John, what makes you different than everyone else out there is that once you have an idea, you actually sit down and do it". To be honest, that never really occurred to me, and it made me stop and really think. I just took it for granted that if you have a great idea, you just do it right?

I think it's time to do an update on good old Jeff Paul and his "Short Cuts to Internet Millions". Jeff Paul has had a little time, since the first review I wrote on him, to either improve the way he runs his online companies or continue to run scams.

Well after running a few searches on Mr. Jeff Paul it looks, to me as if John Paul has not changed at all.

Here are two complaints on Jeff Paul I have found on other sites.

12/5/2008 - Marty writes: I've done a bit more research. Jeff Paul runs this under variouse company names, and the BBB rates them an F. Multiple complaints like the ones listed on this site. Company never responds to these complaints as the BBB has no enforcement authority. The CA Attourny General needs to look at these clowns that have taken so much from so many. I've not found one positive revue of any of his products other than those he's produced himself on his own sites. Testimonials to how great all this crap works. Cancell your credit card ASAP. Even if you try to cancell with these scam artists, they will charge you again!

Having owned, run, and sold other very sucessful companies, I felt a few thousand dollars was a small investment to help my small staff get a couple of good websites up and operational so we could improve our internet presence. Despite our 35 years of other business accolades we had little internet experience or presence. The materials were sophomoric at best and foreshadowed the competence of the coaches assigned to help us. Jeff Paul's idea of canned websites turns out to be reselling junk literature from their portfolio. Not exactly as represented when we were solicited or promised over the phone. Of course they sell your name to web developers who try to get a second and third bite. Offers for dozens of valuable services come often. My staff, despite their competence, have gained little in training or competence, as the Jeff Paul system has proven virtually worthless in producing a true ecommerce website worthy of any companies reputation. The infomercial is appealing and obviously works for them. But if your goal is truly to create value for your company or self by selling on the internet. Save your few thousand (even it that is not a lot of money to you), and find a local web developer with great references and a few years of experience they are plentiful. Price paid, lesson learned. Avoid Jeff Paul.

Well people it looks to me like Mr. Jeff Paul hasn't done a thing to improve his image. In fact it looks like Jeff Paul as continued to scam people right and left.

As I said in my first look at Jeff Paul, "The only thing I can say about Jeff Paul is it would be best, IMO, to stay away from him."

Jeff Paul's "Short Cuts to Internet Millions" does have a low cost startup, but it's just the tip of the iceberg, as you will be solicited for various upgrades, paid training and expensive mentoring After researching Jeff Paul I found tens if not hundreds of complaints about the Jeff Paul system. Many people have paid $1950.00 for the revolutionary system, which claims to provide a 60-day unconditional money back guarantee. Of course the money was never returned. One person was taken for $8,500.00, which was never refunded! This person claims to have tried to contact Greg Wardin the so-called 'confidence man' many times requesting a refund. All attempts failed. In fact he has never even been able to reach Greg Wardin. This doesn't sound much like an unconditional money back guarantee to me. The only thing I can say about Jeff Paul is it would be best, IMO, to stay away from him. - 18098

I enjoy telling people that I "work at home." I can see that wistful look in their eyes, and I can hear it in their voices when they say, "That must be nice." And then for about five minutes, they do a little daydreaming about what it would be like to "work at home." I never explain to them about the measure of self-discipline it takes, and how great are the temptations to take a "break" and reorganize my sock drawer when I find myself faced with working on a project that is less than interesting (or worse yet, "not as profitable" as other projects). Why burst their bubble? Let them daydream awhile?.

Working at home can be all the wonderful things that those people daydream about. Setting my own hours, working at my own pace (project deadlines permitting), running to the market at 1 o"clock on a Wednesday afternoon when there"s nobody there---these are just a few of the plusses of working at home. For parents (whether it is the mom or dad, or both working at home), being able to adjust their working schedule to school schedules, being able to take the kids to the orthodontist or to soccer practice, working at home is a definite plus. For people who don"t work at home and who want to work at home, it is probably hard to think of even one minus. But the fact of the matter is, the minuses do exist---in abundance.

It is not my purpose here to ruin all your hopes or plans to establish yourself as a work-at-homer. In fact, I do encourage you to follow that dream and be your own boss. However, I want you to take just a few minutes to take stock of your situation and once and for all decide if working at home really is in your best interest.

First, and obviously, you have to have a job skill that is amenable to the at-home environment. If you trim poodles on the weekend and think you can turn it into a full-time at-home business, then look around. Do you live in a two-bedroom apartment on the second floor and just do your neighbor"s poodle once a month (but you do it really well)? Or do you live a four-bedroom ranch house with a three-car garage that can be made over into "Donna"s Clip Joint" and happen to live in an urban neighborhood where everyone has pets? And even if you do, what about all the permits that you will need? Will you be able to pursue this type of work at home, and do so legally?

The most applicable work-at-home job skills involve the use of a computer, the Internet, phone, fax, and other small business/office machines. You can hardly drive from home to the grocery store without seeing those signs nailed to telephone poles: "Got a Computer? Earn up to $2000 at home!" Of course, you can, but can you?

For the sake of argument, let"s presume then that you have a computer and an assortment of small business machines, and that you do indeed have some skill that you believe can be marketed. Let"s focus on you for a minute: Are you self-disciplined? Are you organized? Do you have the proper workspace where you can work undisturbed? Are you able to face a workday where you have no supervision or guidance? Can you work at home, day after day, without having the people contact that you would typically find "on the job"? When you have overlapping projects, will you be able to cope with the necessary prioritizing so that you get the time-critical job done on time without jeopardizing the next job in line? - 18098

Do You Need a Bailout?

President Bush has been on the TV a lot lately. Too late for him to go down in history as a good president, but we will give him credit for trying. The Pres. has assured us all that we can grow our economy by spending more money. He even sent us each a few hundred to help us do that. One has to wonder if that was a set up for what was to come.

Next came the BIG bailouts for the banks and boys on Wall Street. Hey, where do we apply for some of that 700 Billion dollar pie? Well, don't hold your breath on that one (in a moment we will show you how to cash in on the bail out actions with simple mini-dow index trading)!

Let's see, if you are already in debt up to your ears - like the US government is, how is sending out free money going to stimulate the economy? And, how is that going to help the US government?

OH, don't forget our friends over at the FED. The Reserve! The agency that is owned by the bankers. That masquerades around like they are part of the government. What many folks still don't know is that they all pulled a fast one on us by sticking that word Federal in front of their name. The same thing the guy at Federal Express did when starting his company.

Frederick W. Smith founded FedEx. I clearly remember years ago when he was on 60 Minutes, he said that by the time folks figured out that he was not part of the government his company was already well on it's way to success! Can't blame his reasoning? What a PLAN! IT WORKED for the FED why not FedEx too?

Let's quote right from the FedEx web site:

"Federal Express was so-named due to the patriotic meaning associated with the word "Federal," which suggested an interest in nationwide economic activity. At that time, Smith hoped to obtain a contract with the Federal Reserve Bank and, although the proposal was denied, he believed the name was a particularly good one for attracting public attention and maintaining name recognition."

I'm sure Smith did want a relationship with the Federal Reserve - who wouldn't! These guys have the legalized right to print money! Think about it. It does not matter if it is a $1 bill or a $100 bill, it cost them about the same to make it (a few cents each). Then they "LOAN" that money at full face value to the US government. Full face value PLUS INTEREST! So now you know where the national debt comes from. We now owe that money - Plus Interest - to the FED. A private corporation controlled by international bankers.

So if you are thinking that Bush's plan to grow the economy by handing out $100 bills won't cost anything - Think Again! Where is that money going to come from? That's right - the good ol boys at the FED. These mystical folks seem to be able to pull money out of thin air! Just think, with today's high-tech world, the FED can just punch a button on a computer somewhere and release new funds to the world. Most of which never represents new bills being printed, but just credit in some bank or financial institutions account. Electronic numbers moving through nanoseconds of time and space.

Not only does the FED create money, they also have the ability to set their own interest rate!

- The Fed's Open Market Committee (FOMC), announces their interest rate decisions. This is NOT the interest rate that you and I can get money for, (why don't we all meet at the Fed Discount Window - wherever that is) but what the BIG boys who keep the whole world flowing receive. They in turn pump up the volume and pass the savings on to you and me right - WRONG! It could take weeks or even MONTHS after a cut to see any savings at the consumer level. So why do the markets get so active after an FOMC announcement?

The BIG boys are the ones who really move the market right (and they CAN line up at the FED window for a bailout). We just want a small slice of it. That's all. Remember that when you are trading (or practicing the FED move trade -after an FOMC announcement).

So how do you cash in on the bailouts without getting a slice of the pie? Index trading! With all these bailout moves, the FED buying stock and giving away billions of dollars, it has caused some GREAT moves in the market. Not so good for stock traders, but Wonderful for those of us that just trade and follow the overall index.

No matter what happens, we can all do well with Simple Mini-Dow Index Trading. I look for GREAT times ahead for Index traders. We might have to pay more for the things we need, (because of the FED printing out bailout money like candy these days) but at least we can stay home and earn the money to get them!

Remember those FOMC announcements mentioned earlier? Many times after an announcement, the market moves and moves BIG. Much like the market moves we have all been seeing here lately with the bailout manipulation of the markets. The FED won't give you a partnership deal like FedEx was looking for, but you can capitalize on their dealings.

You may not be able to get in line for a bailout, but you can stick your hand in the market and cash in on the Wild moves we are now seeing in the FED manipulated market. Just follow an index and stay away from stock! - 18098

I am and Internet marketing newbie. Each day I get two or three get rich quick schemes in my e-mail box. They all sound fabulous and I'm sure they work for the person who originally designed the program. I have no doubt that the testimonials on the professionally written pages are genuine.

Lots of us are drawn in by the easy money and get rich quick sales copy. I don't want to travel down that easy money road because I don't believe it will work for me.

Get rich quick schemes and multilevel marketing don't interest me. I want to be involved in a business that will teach me what I need to know to be an online 'survivor' - something that gives good value for the money and is a long-term proposition.

What I hope to find are people who would like to share information on the programs they are involved in. I don't mean to get the program for free- that wouldn't be ethical - but I would like to know about programs that other people think highly of.

Is the content being explained clearly? Is the teacher of the course accessible - or after you've paid is he/she very difficult to contact? Have you been able to successfully implement the program? Can the program work in a wide variety of niches? To successfully implement the information taught in the program - how Internet savvy do you need to be? What tools and resources (website, autoresponder, SEO expertise, video creation/editing, etc.) do you need to make the information 'work'?

Also - if you tried a program - genuinely tried to make it work but failed - I'd like to know why. Was it something in the program that was missing? Or was it simply not a good fit for you? Etc. We can learn as much from our mistakes as our successes. We don't fail until we stop trying.

I'm going to post the video on my blog every day about my progress through a business system that I have purchased. It will give you a chance to see what this system has to offer without actually having to pay for it and, hopefully, you will share information with others who visit my blog about a program you are involved in. Last year, I bought the Eyeclops bionic eye for my son, and he thought it was pretty cool. When I first heard that EyeClops Night Vision Goggles were coming out, I was very excited, for myself AND for my son!

Honestly, at first I was disappointed with how they worked, until I asked about traditional night vision goggles and realized that night vision has its limitations, no matter how much you spend.

The craziest thing is that Jakks Eyeclops Night Vision Goggles are $80. Can you believe it? If I hadn't bought from Eyeclops before, I would think that they're junk, but since I was really happy with the previous toy from them, I went ahead and bought them.

Since the eyeclops night vision goggles only provide a small field of vision, and due to the fact that your night vision is only seen through the right eye, your depth perception will not work as well either. Any communication, regardless of the medium, initiated for the purpose of advertising availability or quality of any property, goods, or services, but such term does not include a communication (A) to any person with that person's prior express invitation or permission, (B) to any person with whom the party has an established business relationship.

The benchmark is a standard or point of reference and in terms of measuring marketing performance, benchmarks are crucial. Benchmarking is the better marketing measurement campaign that aims to use measurement to prove the link between marketing and profit. In the case of education, measurement techniques can be used to prove the link between marketing and student recruitment, external income generation, or whatever the marketing activity has set out to achieve.

However, the task of measuring marketing performance is not an easy one marketing activities take place at a number of different levels, operational, tactical and strategic. In the case of the latter the effects are longer term and less easy to monitor. Proving the link between marketing and profit is not the only reason for measuring marketing performance. If you do not know how effective different activities or initiatives have been then it is not possible to make improvements and ultimately increase value for money with regard to marketing expenditure and deployment of resources.

The task for marketers is to consider how can we measure the effectiveness of what we are doing? The starting point of any measurement has to be establishing a benchmark. From my experience of working within education marketing, increasing numbers of marketers are investigating marketing performance at a tactical level by measuring the effectiveness of specific and discrete campaigns using response mechanisms built into the promotion or admissions procedures. It is important to establish the baseline at the start what would have been achieved without the activity? In the case of introducing new techniques for an annual campaign, the benchmark will be the level of enquiries and/or numbers of students enrolled in the previous year(s).

However, in evaluating effectiveness, other aspects which may impact upon changes in response must also be noted and taken into account. Other marketers are starting to look more broadly at the range of activities that impact on success in terms of student recruitment. A strategic approach to portfolio management, enquiry management systems, improved relationship marketing and better targeting of marketing activities can result in reductions in the numbers of course failures, increasing share of applications, improved conversion, reduced spend in Clearing, achievement of target numbers and reductions in drop-outs.

The difficulty comes in proving the links. Crucial to this is the auditing of current activities and performance levels. The institution needs to be clear on how it is currently performing, and this needs to be built up from school/department level as activities and performance will probably differ. This will determine the current position from which to evaluate future performance. - 18098

The UK is going to reduce its carbon emissions by eighty percent by 2020, compared to 1990 levels - according to the government. In spite of this statement, UK people are questioning their government's dedication to wind power.

Wallstreetpit.com has recently published an article that claims the wind power projects owned by BP in the UK, Turkey, China and India will shut down and BP will refocus its energy on the US. This same article suggests that thirty percent of the UK's energy supply will come from wind power. Readers are wondering why the government is letting BP move their wind power projects to the US when they claim to be focusing on green energy sources like windfarms.

Adding a few questions of its own, the Guardian website released an article claiming that Great Britain is one of the best locations in the world for windfarm technology. Great Britain's lengthy coastline and famous wind conditions lend it nicely to the development of windfarms.

How Much Should You Pay the Wedding Minister?

Now you might ask, should I forget about drop shipping? My answer is NO. If done right, dropshippping could be a very effective strategy at least for jump starting your business. My advise is, if you are just starting out, and you don't have a lot of money up front, consider to drop ship your products. When you do this you will also be able to see how popular differnet products are. On ebay you can tell by the number of bidders. On comparison shopping sites, you can look at their popular products sections, which will give you an idea about what consumers are buying.

In conclusion, study your market and choose your products before you begin selling a single item. If you are on a shoe string budget, then consider drop shipping your products using reliable and legit wholesale suppliers.

If you have some capital to invest I would advice buy bulk wholesale. You will get better prices for actually buying the merchandise. The online marketing scene is hard! Don't for one instant believe the adverts that promise you bucket loads of cash whilst only working part time each month. It simply is not true.

You MUST set up a working schedule for yourself. You must adhere to that schedule and you had best remember that quitting time is when you get through and not precisely eight hours after you went to work.

Western employers were found to be more considerate and are more likely to treat their domestic helpers equally and allow them more personal space, thus giving the helpers more work satisfaction.

Western employers are more homogeneous in terms of variations in treatment of filipino domestic helpers and also tend to concentrate on the positive side while Chinese employers are more diversified. As a summary, filipino domestic helpers are more likely to know what Western employers are expecting of them and how they would be treated versus the Chinese employers where treatment usually vary from one employer to the other. - 18098
